  1. Client Services 

Effective Pain Solutions provides pain education services to participants who have had attendance approved under a third-party claim or have agreed to attend and fund the service privately.

 
  1. Fees for Services

  • (a) Effective Pain Solutions will charge your third-party payee for services provided if participation is approved by a third-party.
  • (b) If participation in the program is not covered by a third-party the fees will be paid by you, the participant, according to the agreed fee structure determined prior to entering the program Additional expenses or services will require approval from a third-party payee or are your responsibility and to be paid by you.
  • (c) Additional expenses outside of the third-party agreement or services will require approval from a third-party payee, or are your responsibility and will be paid by you the participant
  • (d) The fees for the services provided by Effective Pain Solutions are provided to third-party payees or to the private individual at time of initial enquiry.
  • (e) Once an approval and/or an agreement to attend a program is confirmed and agreed upon, prices will not change for the entirety of the program.
 
  1. Payments

Effective Pain Solutions will be paid in respect of Services provided by one of the below payment methods:

 
  • (a) Third-Party Payee – Effective Pain Solutions will forward an invoice for Services completed to the appropriate third party such as CTP insurer. This must be paid within two weeks (14 days) of the invoice date;
  • (b) Individual – Effective Pain Solutions will send the invoice for services to you for payment which must occur within the terms negotiated in your payment plan. The payment plan will be negotiated, agreed upon and documented prior to the commencement of the service.
 

4. Your Rights and Responsibilities

During the term of the program, Effective Pain Solutions will:

 
  • (a) act with respect for your individual rights to freedom of expression and self-determination in decision-making, in accordance with applicable laws and conventions,
  • (b) Respect an individual’s right to privacy,
  • (c) Provide Services in a safe and competent,
  • (d) Act with integrity, honesty and transparency,
  • (e) Promptly take steps to raise and act on an individual’s concerns about matters that may impact the quality of services, or an individual’s safety.
 
  1. Responsibilities of the Client/Participant

You and your Representatives agree to

 
  • (a) Advise Effective Pain Solutions about any concerns that arise about the services provided,
  • (b) Be an active participant within the program,
  • (c) Ensure that any absences be reported so provision of additional services, if required may be arranged, and content from the non-attended Service such as recordings, will be provided as soon as possible.
  • (d) Show respect for other participants and facilitators at all times
  • (e) Ensure any absences are reported so provision of additional services outside of the program dates and times may be arranged; content from the non-attended session may then be provided via session recordings or arranged additional consultations.
 
  1. Cancellation / No Show Policy and Termination

Effective Pain Solutions reserves the right to charge for non-attendance of the approved program. If for any reason, a participant decides to cease attending the program within the first 6 weeks, payment for half of the programs total cost will be invoiced for payment. If a participant ceases the program after week 6 the full program cost will be invoiced. The decision to terminate participation in the program by a participant or other must be communicated in writing as soon as possible to reduce the risk of overbilling of services. Effective Pain Solutions will make every effort to ensure full program attendance is achieved by all participants. Supplementary services such as videoconferencing is available if required. In addition, one-on-one consultations and provision of all recordings and program materials can be provided to ensure participants have the best chance of achieving success during and after the program.

 
  1. Privacy

  • (a) Participant privacy is extremely important to Effective Pain Solutions and a mandatory requirement of all health service providers. During the program we will collect and use personal contact details and information provided by you to assess psychosocial health and track progress throughout the program.
  • (b) Any information you provide, or we receive as part of a referral, either verbal or written will be stored securely to protect individual privacy. This information may be discussed with referring health care providers or may be used in discussion/reporting to third-party providers during and following the program.
  • (c) Respect and protection of privacy of all participants information must be observed by both facilitators and participants within the program. Any participant who is seen to misuse information disclosed within the program is a breach of another participant’s privacy. The offender will be asked to leave the program and full program payment will be collected.
  • (d) Our Privacy Policy defines how personal and sensitive information will be collected, used and disclosed. Please read the Effective Pain Solutions Privacy Policy carefully as it contains important information relevant to an individual’s expected conduct within the program. Consent to the disclosure, collection, and use of personal information is not mandatory and participants have the right to disclose whatever information they feel comfortable providing.
 
  1. Feedback and Complaints

(a) Effective Pain Solutions encourages any feedback, concerns or complaints during the program and post program. This provides the opportunity to address and resolve any participants concerns immediately and for us to improve our services.(b) You can make a complaint to Effective Pain Solutions

  1. Feedback and Complaints

Effective Pain Solutions services incur a Goods and Services Tax therefore, this will be indicated on the invoice for services provided.
